1. This is to report the loss of a trunk that I had booked form New Delhi to Mumbai Central in the brake van of Rajdhani Deluxe/Air Conditioned Express that left New Delhi station on ……………… (date) ……………… a.m. /p.m.

2. This is to report the disappearance of a bicycle booked by me from Delhi to Mumbai in the luggage van of the Frontier Mail which left New Delhi station on ……………… (date) ……………… a.m. /p.m.

3. This is to inform you that I have lost a basket of fruit I had booked in the luggage van of Rajdhani Express leaving Delhi for Mumbai on ……………… (date) ……………… a.m. /p.m.. I had also travelled by the same train.

4. The luggage I had booked in the Delhi-Howrah Express on the 15th of this month could not be traced at the destination.

Details of the lost article/luggage;

5. I myself saw it being labeled and put in the brake van which was booked vide receipt No.R/ 54321. But at Mumbai Central it was missing.

6. I have the booking receipt of my Atlas bicycle.

7. The said fruit basket, for which I have the booking receipt number R/368325 contained 3 dozen apples of the ‘Delicious red’ variety. My name and Mumbai address had been boldly written on its white cloth cover.

8. The said luggage booked under RR no.43913 was insured and clearly marked for delivery at the destination, Howrah.

Stressing an early tracing of the missing article:

9. Shall be highly obliged to you, if you could take a prompt action in this regard, as the trunk contains some important documents/valuables/medicines.
