I have the honour to inform you that my original identity card has unfortunately been lost. Because of the missing of the identity card, I am facing several inconveniences. I am unable to get permission into the library, reading room and the journal section. I am also deprived of the travelling concession for the same reason. I am a poor boy and I can't pay high fare regularly.

Therefore, I request you to issue a duplicate identity card. I herewith furnish a copy of my passport size photograph.
